The Initial Eligibility Aggregate Performance form documents verifiable program-specific performance information for training programs seeking first-time inclusion on Indiana's INTraining and Eligible Training Provider List (ETPL). Under WIOA §122 and 20 CFR 680.450, new programs without sufficient completer data may receive a one-year initial eligibility period while performance data is collected.
What the form includes
- Program title matching the ETPL / INTraining listing
- CIP and SOC codes, duration, and credential description
- Alignment with in-demand occupations and business partnerships
- Aggregate performance metrics or new-program attestation (no completer data yet)
- Authorized preparer attestation
